Hi Sarah - not sure exactly which type of form you have set up. It might be worth testing a few different types (Modal, Inline, Slide-in) and templates until you get something that works better with mobile.
I think the way to do this is to set up multiple products. So you would have one product for the Yearly subscription and another separate product for the monthly option.
